The Neighbourly Nourishment Program is the cornerstone of DuMonde Helping Hands Foundation’s mission to combat food insecurity within our communities (Black & Asian). Food Bank Waiver and Release of Liability You MUST agree to this waiver if need to become a recipient of DuMonde Food Bank, I, the undersigned, agree to the following terms and conditions while receiving food and related items from DuMonde Helping Hands Foundation (“the Foundation”) through its Neighbourly Nourishment Program: 1. Acknowledgement of Voluntary Participation I acknowledge that my participation in the food distribution program is voluntary, and I am accepting the food and items as-is, with no guarantees or warranties regarding their quality, safety, or suitability for consumption. 2. Assumption of Risk I understand that the food items provided may include perishable goods or products nearing expiration, and I assume all risks associated with the handling, storage, and consumption of such items. 3. Release of Liability I release, indemnify, and hold harmless DuMonde Helping Hands Foundation, its staff, volunteers, agents, and affiliates from any and all liability, claims, or damages that may arise from the use or consumption of food or items received, including but not limited to foodborne illness, allergic reactions, or injuries. 4. Prohibition of Resale I understand and agree that the food and items received are for personal use only and may not be resold, distributed, or used for commercial purposes. 5. Acknowledgment of Terms By signing below, I acknowledge that I have read, understood, and agree to these terms. I understand that failure to comply may result in loss of access to the program.
